Who runs this site

What we collect
Only what you type into one of the two forms on this site, and nothing about your health.
- Physician request form (For providers page): your email address, your name if you give it, and a hidden field that marks the request as coming from a physician or practice.
- Patient form (Ask your doctor page): your email address, your ZIP code, a practice name if you give it, and a hidden field that marks the request as coming from a patient.
- With either form, the title of the page you sent it from.
Please do not send health information through either form. This site does not ask for it and is not designed to receive it.
Why we collect it
- Physician form: to contact you about early access to AEGIS for your practice.
- Patient form: to contact you about practices that offer AEGIS near you. We do not contact your doctor on your behalf without a separate request from you.
Florida only
AEGIS is available through Florida practices for now. The patient form accepts Florida ZIP codes only. If a submission from outside Florida reaches us anyway, we delete it on receipt.
Who else receives it
- Formspree, which receives form submissions and forwards them to us. Formspree acts as our processor for this purpose.
- Google Fonts. Every page loads its typeface from Google’s servers, so your browser sends Google a request that includes your IP address and browser details. We do not control what Google does with that request.
- Netlify, which hosts this site and records standard server logs (such as IP address, page requested and time) when you visit.
AEGIS does not sell the information collected through this site.
Cookies and tracking
AEGIS sets no cookies on this site and uses no analytics or advertising trackers. We do not track you across other websites, and no third party is permitted to do so through this site, apart from the requests described above. Because we do not track you, the site responds the same way whether or not your browser sends a Do Not Track signal.
How long we keep it

Your choices
You can ask to see, correct or delete what you sent us by writing to privacy@aegiscis.com. We will act on the request and confirm when it is done.
Children
This site is not directed to children, and we do not knowingly collect information from children under 13.
Changes to this notice
If we change this notice, we will post the new version on this page with a new effective date. If a change is material, we will say so at the top of the page.
